Communities In Schools of San Antonio was established in 1985 to address our city's high dropout rate. The founders' vision was to provide a positive environment for at-risk students, insuring their success to educational opportunities, social services and support services. We are seeking a passionate, committed, and value oriented individual for the position of Case Manager.  The Case Manager will be part of an team oriented staff and must possess professional level knowledge.

 

Job Description

 

The Case Manager working out of the Haven for Hope campus will provide case management services and support to high-risk students experiencing homelessness with the intent to increase attendance rates and reduce behavioral and disciplinary problems in school. This is a collaborative partnership with Haven for Hope to meet the high needs of students residing at Haven for Hope.

 

Minimum Requirements 

  • Master’s degree in a social work or counseling

  • Minimum two years of experience with youth and families in the social work, counseling or case management field (including graduate internship hours)

  • Experience with at-risk populations and cross-cultural environments

  • Knowledge of client groups and/or issues related to the population

  • Ability to work within a multi-disciplinary team

  • Ability to set priorities and work with minimal supervision

  • Strong communication skills with an ability to articulate program goals in a public setting

  • Possess a valid Texas Driver’s License and have reliable transportation

  • Ability to pass a fingerprint based background check under TEA regulations

Preferred Qualifications

  • Bilingual ability in Spanish and English preferred

 Major Responsibilities

Intake & Assessment

  • Complete intake and conduct initial assessment with referred students and families
  • Create a plan for services to address student and family needs
  • Communicate with appropriate school staff on a regular basis to seek input and report on student progress, case status, and referral needs

Service Provision

  • Provide case management services to program participants in coordination with the CIS model for service delivery
  • Enroll and provide individualized support services to all school age students who are living at Haven for Hope who will be tracked for progress
  • Provide crisis intervention during critical situations
  • Collaborate with appropriate school staff that focus on student support services and act as an educational advocate for participants
  • Collaborate with Site Coordinators at CIS campuses to provide services to participants
  • Monitor and document student progress toward goals
  • Gather parent feedback and provide support to include parent education and health and social service referrals

 

Collaborative Efforts

  • Serve as a liaison to student's home school 
  • Work closely with Haven for Hope staff and schools as appropriate to identify the highest risk students in need of support;
  • Coordinate with partners and service providers to address the needs of the student; and
  • Provide relevant referrals to clients for on-going support to meet individualized needs
  • Coordinate and oversee interns assigned per university requirements
  • Provide intern orientation regarding CIS program policies and school-based service provision
  • Attend weekly staffing meeting with program staff
  • Research and become familiar with other programs and resources in the community that might benefit participants and their families

Data Maintenance and Evaluation

  • Complete paperwork required for grant compliance
  • Maintain service logs in accordance with program procedures and guidelines
  • Document services from service logs in the CIS approved data entry system
  • Implement, manage, and report on grants
  • Evaluate program effectiveness and provide bi-annual performance report for participants
  • Gather and maintain educational data on participants

 Administrative

  • Manage time card
  • Attend all meetings and trainings as required
  • Ensure the completion of required programmatic reports
  • Plan appropriate expenditures and maintain budgets
  • Participate in relevant professional development activities to increase knowledge and skills

Position Type/Expected Hours of Work

 

This is a full-time position. Day and hours of work are generally Monday through Friday between 8:00 a.m. to 6:00 p.m. with some evening and weekend hours required.
