Communities In Schools of San Antonio was established in 1985 to address our city's high dropout rate. The founders' vision was to provide a positive environment for at-risk students, insuring their success to educational opportunities, social services and support services. We are seeking a passionate, committed, and value oriented individual for the position of Re-Engagement Case Manager.  The Re-Engagement Case Manager will be part of an team oriented staff and must possess professional level knowledge.

 

Job Description

 

The Re-Engagement Case Manager will coordinate the provision of social services within the Re-Engagement Center. The target population will be opportunity youth who face obstacles to personal, academic success and career success. ACEs (Adverse Childhood Experiences) and Trauma Informed data and tools will be used as a basis to provide wrap around support to the opportunity youth.

 

Minimum Requirements & Preferred Qualifications

  • Minimum Master’s Degree in Social Work and 2 years’ related work experience
  • Master Social Work License preferred (LMSW)
  • Excellent communication skills, verbal and written.
  • Knowledge of client groups and/or issues related to the program area.
  • Proficiency in the use of computers for: word processing, databases, spreadsheets, e-mail, internet.
  • Experience with at-risk populations and cross-cultural environments.
  • Proven ability to set priorities and work with minimal supervision.
  • Ability to articulate program goals in a public setting.
  • Experience working with youth and families.
  • A willingness to follow the philosophy of CIS and operate as a team player

  • Possess a valid Texas Driver’s License and have reliable transportation.

  • Bilingual ability in Spanish and English preferred

 Major Responsibilities

Case Management Support

  • Implement an intake and assessment process with each opportunity youth to establish a baseline for services to include using the ACEs Assessment tool.
  • Assess and create Individual Service Plans (ISP) to provide or coordinate services to help navigate and enable the opportunity youth to re-enter academic tracks and/or enter post-secondary education, employment or training.
  • Develop and implement individual and group Trauma Informed services that address the individualized youth assessed needs.
  • Connect opportunity youth to the necessary community resources that meet their assessed needs.
  • Provide follow-up services and track performance of youth throughout case management process.

 Program Operations

  • Assure the adherence to all contractual and grant provisions and any assigned budgetary requirements.
  • Implement, manage and report any needed information for grants and projects assigned to program.
  • Meet contract obligations by obtaining qualified caseload and goals by prescribed deadlines.
  • Maintain quality of programming and submit accurate required reports.
  • Keep abreast of program developments, changes and updates from Lead Re-Engagement CM Coordinator, Re-Engagement Center and CIS.
  • Communicate any updates, successes and any pertinent information in job responsibilities to the Lead Re-Engagement CM Coordinator.
  • Facilitate and coordinate transition of the opportunity youth to assigned school by connecting them to a CIS Site Coordinator or School Liaison.

 Integrative Services

  • Broker with outside agencies, businesses, volunteers and interns to provide direct services or referrals that meet student needs.
  • Assist Re-Engagement partners with client success through collaborative case management support to meet individual goals.
  • Maintain strategic collaborations, established for the Re-Engagement Center; with businesses, universities, community organizations to create opportunities for the opportunity youth.
  • Coordinate and effectively communicate with partners to insure clear collaboration and integrative services to ensure success of program. 
  • Provide volunteer and community partner orientation regarding CIS & Re-Engagement program policies.
  • Coordinate in-kind donations to support student needs and program activities.

File and Data Maintenance

  • Follow polices and procedures to all aspects of documentation in a timely manner.
  • Collect CIS-SA paperwork, demographic and other necessary information for each identified youth and enter this data into the approved data entry system.
  • Enter Individual Service Plan (ISP) and services in the approved database system.
  • Establish, document and maintain accurate and complete youth files with appropriate and up-to-date information/documentation as per program procedures and guidelines.
  • Obtain sign-in sheets for services in accordance with program procedures and guidelines.

  Administrative

  • Manage time card
  • Attend all meetings and trainings as required
  • Ensure the completion of required programmatic reports
  • Plan appropriate expenditures and maintain budgets

Position Type/Expected Hours of Work

 

This is a full-time position. Forty-hours per week with alternating work schedule, some evening and weekend hours required.
